Make home visits to clients after hours and on weekends
Perform medication administration, wound care, treatments, and comfort care
Monitor client conditions and report changes to the Clinical or Client Services Manager
Follow up on physicians’ orders and document care
On-call 5:00 PM–8:30 AM, rotating weekly
Serve Windsor, Orange, and Grafton counties in VT and NH

🎯 Requirements

Current RN license in Vermont and New Hampshire
Minimum of one year of recent clinical (medical/surgical) experience
Prior home health and/or hospice experience strongly preferred
Solid computer skills; EMR experience preferred
Ability to travel to assigned cases
On-call availability for evenings and weekends
