Deliver skilled nursing care at home for adults and geriatric patients
Complete comprehensive assessments and follow physician orders
Perform medication management, wound care, disease management, and patient education
Accurately document care and communicate with the interdisciplinary care team
Support patient independence, safety, and quality of life at home

🎯 Requirements

Active RN license (Compact license required or PA license)
Willlingness to do home visits in Philadelphia
Minimum 1 year RN experience (home health or med-surg a plus)
Strong clinical judgment and time management
Compassionate, dependable, patient-centered
