Making home visits to clients in designated geographic territories.
Administer medication, wound care, treatments, and comfort care.
Monitor client conditions and report changes to the Clinical/Client Services Manager.
Follow up, execute, and document doctors' orders.
Perform client assessments as necessary.
Occasional on-call after hours and weekends.

🎯 Requirements

Current RN license in VT & NH.
Minimum 1 year recent clinical (medical/surgical) experience.
Home care or hospice experience preferred, not required.
Solid computer skills; EMR experience preferred.
Ability to travel to cases as assigned.
