Follow a designated care plan per patient needs.
Conduct home visits to clients in designated territories.
Administer medications, wound care, treatments.
Monitor clients' conditions; report changes to Clinical/Client Services Manager.
Document doctors' orders and ensure proper follow-up.
Perform client assessments and contribute to care planning.

🎯 Requirements

Current New Jersey RN license.
Minimum 1 year clinical (medical/surgical) exp.
Prior home health experience preferred.
Proficient with EMR/electronic documentation.
Strong interpersonal and time-management skills.
Ability to travel to assigned cases.
