Conduct home visits per patient care plan in Hazleton area.
Provide 1:1 home care for clients.
Administer medications, wound care, and treatments.
Monitor client conditions and report changes to the Clinical or Client Services Manager.
Document doctors' orders and care interventions.
Perform client assessments and care coordination.

🎯 Requirements

Current Pennsylvania RN license.
Minimum 1 year of recent clinical (medical/surgical) experience.
Graduation from an accredited nursing program.
Ability to work independently and manage time effectively.
Strong interpersonal skills; EMR experience preferred.
Ability to travel to cases as assigned.
