Follow a designated care plan according to patient needs
Make home visits to clients in designated geographic territories
Perform medication administration, wound care, treatments, and procedures
Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors' orders
Perform client assessments and care management activities

🎯 Requirements

Current TN RN license
Minimum 1 year clinical (medical/surgical) experience
Graduation from an accredited nursing program
Prior home care experience preferred but not required
Ability to work independently and manage time
Strong interpersonal and computer/EMR skills
