Follow designated care plans per patient needs
Conduct home visits to clients in assigned territories
Administer medications, wound care, treatments, and procedures
Monitor client conditions and report changes to the Clinical or Client Services Manager
Document doctors' orders and ensure proper follow-up
Perform client assessments and document observations

🎯 Requirements

Active Pennsylvania RN license
Minimum 1 year recent clinical (medical/surgical) exp
Graduation from accredited nursing program
Ability to work independently and manage time
Strong interpersonal skills
Solid computer skills; EMR experience preferred
