- Follow designated care plans for patient needs in ME home health.
- Make home visits to clients in ME territories (Yarmouth, Freeport, Falmouth, Cumberland).
- Administer medications, wound care, and treatments per orders.
- Monitor client conditions and report changes to Clinical or Client Services Manager.
- Document observations and interventions using a tablet EMR.
- Coordinate case management and communications with the care team.
🎯 Requirements
- Current Maine RN license.
- Minimum 1 year clinical (medical/surgical) experience.
- Home care experience preferred but not required.
- Graduation from accredited nursing program.
- Ability to work independently and manage time.
- Strong interpersonal skills; EMR experience preferred.
