Follow designated care plan per patient needs
Make home visits to clients in designated geographic territories
Administer medications; wound care and treatments
Monitor client conditions; report changes to Clinical or Client Services Manager
Follow up with, execute, and document doctors' orders
Perform client assessments; case management and coordination

🎯 Requirements

Current VA RN license.
Minimum 1 year clinical (medical/surgical); home care preferred.
Graduate of accredited nursing program.
Ability to travel to cases; work independently; strong time management.
Strong interpersonal skills; EMR experience preferred.
