Follow a designated care plan in accordance with patient’s needs
Make home visits to clients in designated geographic territories
Administer medications, wound care, treatments, and procedures
Monitor clients' conditions and report changes to Clinical or Client Services Manager
Document doctors' orders and follow up as needed
Perform client assessments as necessary

🎯 Requirements

Current NH RN license
At least 1 year of clinical (medical/surgical) experience
Graduation from accredited nursing program (transcript/diploma)
Ability to work independently and manage time effectively
Strong interpersonal skills
Solid computer skills; EMR experience preferred
