Follow designated care plans per patient's needs.
Make home visits to clients in designated territories.
Administer medications, wound care, treatments and procedures.
Monitor client conditions; report changes to Clinical or Client Services Manager.
Document doctors' orders and interventions accurately.
Perform client assessments and coordinate case management.

🎯 Requirements

Current NH RN license.
Minimum 1 year clinical (medical/surgical) experience.
Home care experience preferred but not required.
Graduate from an accredited nursing program.
Ability to work independently; travel to cases as assigned.
Solid interpersonal skills; EMR experience preferred.
