Follow designated care plan per patient needs
Make home visits to clients in designated territories
Administer medications, wound care, treatments and procedures
Monitor client conditions; report changes to the Clinical or Client Services Manager
Follow up with, execute, and document doctors' orders
Perform client assessments and coordinate case management with a tablet

🎯 Requirements

Current AZ RN license
At least 1 year clinical (medical/surgical) experience
Home health experience preferred
Graduation from accredited nursing program
Ability to work independently and manage time
Proficient computer skills; EMR experience preferred
