Follow a designated care plan per patient needs
Make home visits to clients in designated territories
Administer medications, wound care, treatments, and procedures
Monitor client conditions and report changes to the Clinical or Client Services Manager
Follow up with, execute, and document doctors' orders
Perform client assessments and coordinate care using a touch pad tablet

🎯 Requirements

Current Vermont RN license
Minimum one year of recent, verifiable clinical (medical/surgical) experience
Graduation from accredited nursing program
Ability to work independently and manage time effectively
Solid computer skills; EMR experience preferred
Ability to travel to cases as assigned
