Follow a designated care plan per patient needs
Make home visits to clients in designated territories
Administer medications, wound care, treatments and procedures
Monitor clients conditions; report changes to Clinical or Client Services Manager
Follow up with, execute, and properly document doctors orders
Perform client assessments and care coordination

🎯 Requirements

Active Vermont RN license.
Minimum 1 year clinical (medical/surgical) experience.
Home health experience preferred, not required.
Graduation from an accredited nursing program.
Ability to work independently and manage time.
Strong interpersonal skills; EMR experience preferred.
