Follow a designated care plan per patient’s needs
Make home visits to clients in designated territories
Administer medications, wound care, treatments, and procedures
Monitor client conditions and report changes to Clinical or Client Services Manager
Document doctors' orders and care activities
Use a touch pad tablet to document observations and actions

🎯 Requirements

Active Vermont RN license
Minimum 1 year clinical (medical/surgical) exp.
Prior home care experience preferred but not required
Graduation from an accredited nursing program
Strong interpersonal and time-management skills
Solid computer skills with EMR experience preferred
