Visit clients in their homes to support recovery after illness, injury, or surgery.
Case manage care plans and deliver skilled care including assessments, meds, and wounds.
Help clients progress from discharge to meeting goals at home.
Provide education to patients and families.

🎯 Requirements

Active and current RN license
One year of clinical experience preferred
Home health experience welcomed, but not required. We will train you!
Ability to travel locally to cases as assigned
