Follow designated care plans per patient needs.
Make home visits to clients in Duval, Baker, Clay, Nassau.
Administer medications, wound care, treatments, and procedures.
Monitor client conditions and report changes to Clinical/Client Services Manager.
Follow up with and document doctors' orders.
Perform client assessments and coordinate case management.

🎯 Requirements

Current Florida RN license.
Minimum 1 year recent clinical (medical/surgical) experience.
Graduation from accredited nursing program (transcript or diploma).
Ability to travel to cases as assigned.
Ability to work independently and manage time effectively.
Solid computer skills; EMR experience preferred.
