Follow a designated care plan per patient needs.
Make home visits to clients in designated territories.
Administer medications, wound care, treatments, and procedures.
Monitor clients' conditions; report changes to Clinical or Client Services Manager.
Follow up with, execute, and properly document doctors' orders.
Perform client assessments and care coordination as needed.

🎯 Requirements

NJ RN license active and current.
Minimum 1 year of recent clinical (medical/surgical) experience.
Graduation from an accredited nursing program.
Ability to travel to cases as assigned.
Ability to work independently and manage time effectively.
Strong interpersonal skills; solid computer skills with EMR preferred.
