Complete comprehensive assessments and follow physician orders
Perform medication management, wound care, disease management, and patient education
Accurately document care and communicate with the interdisciplinary care team

🎯 Requirements

Active RN license (Compact license required or PA RN license)
Willingness to travel/home visits in Philadelphia
Minimum 1 year RN experience (home health or med-surg)
Strong clinical judgment and time-management skills
Compassionate, dependable, and patient-centered
