Referral Coordinator - Queens

📋 Description

• Increase referrals for Forge Health in Queens and surrounding areas.
• Execute BD plans to expand Forge Health referrals with hospitals, PCPs, and partners.
• Build and maintain strong relationships with providers, community orgs, and families.
• Participate in community events, county meetings, and coalitions to promote Forge Health.
• Plan/host events, office tours, and forums to educate patients and referrers.
• Collaborate with internal teams on outreach messaging and alignment.

🎯 Requirements

• 2+ years in behavioral health outreach, provider relations, or business development.
• Knowledge of behavioral health referral processes and provider workflows.
• Availability to work outside standard 9–5 hours and travel as needed.
• Experience building professional relationships with clinicians or healthcare organizations.
• Familiarity with CRM systems and tracking outreach activity.
• Valid state license and access to a vehicle.
