Referral Coordinator - Long Island

📋 Description

• Increase referrals within assigned territory for Forge Health/KPC.
• Execute BD strategies to deepen the referral base with hospitals, PCPs, and BH providers.
• Build relationships with providers, community, government, and families for advocacy.
• Participate in community events and coalitions to promote KPC services.
• Plan and host events to inform patients and referral sources about KPC.
• Create outreach initiatives via phone, email, newsletters, notes, and events.

🎯 Requirements

• 2+ years in behavioral health outreach, provider relations, or biz dev.
• Knowledge of behavioral health models, referral processes, provider workflows.
• Availability to work outside standard hours with travel as needed.
• Experience building relationships with clinicians, practices, or health orgs.
• Familiarity with CRM systems and managing outreach pipelines.
• Valid state license and access to a vehicle.
