PT Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility and safety assessments (ROM, gait, balance, transfers).
• Implement wound care and protective interventions.
• Develop and implement individualized care plans (manual therapy, gait).
• Educate clients and families on rehab and safety.
• Document observations and interventions using EMR.

🎯 Requirements

• Current VA Physical Therapist license.
• Minimum of one year PT experience.
• Graduation from an APTA/CAA/AMA approved program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
