📋 Description

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and safety assessments.
• Provide wound care and protective interventions using evidence-based dressings.
• Develop individualized PT care plans (manual therapy, gait training, balance).
• Educate clients and families on rehabilitation to promote independence.
• Document observations and interventions in EMR with a touchpad tablet.

🎯 Requirements

• Current VA Physical Therapist - PT license.
• Minimum of one year of PT experience.
• Graduate of an APTA-approved PT program (as indicated by transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
