📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility, strength, balance, and safety.
• Implement integumentary protective interventions to prevent wounds and promote repair.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary care plan as appropriate.
• Educate clients and family in rehabilitation care and activities to promote health and independence.

🎯 Requirements

• Current VA PT license.
• Minimum one year PT experience.
• Graduate of an APTA- or AMA-accredited PT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
