- Make home visits to clients in designated geographic territories.

- Continually assess and revise the PT care plan with nursing in the multidisciplinary care plan as appropriate.
- Educate clients and family in rehabilitation care and activities to promote health and independence.
π― Requirements
- Current VA PT license.
- Minimum one year PT experience.
- Graduate of an APTA- or AMA-accredited PT program.
- Ability to work independently and manage time.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; EMR experience preferred.
