📋 Description

• Own outreach, engagement, and care coordination for women in ECM.
• Direct outreach through community groups and shelters.
• Build partnerships with community organizations serving women.
• Manage a small team of community health workers.
• Ensure warm handoffs from outreach to enrolled care.
• Track enrollment, engagement, and outcomes with data.

🎯 Requirements

• 3+ years direct outreach with women in safety-net or community health.
• 2+ years managing frontline staff in health/social services.
• Ability to build trust with women facing homelessness, SUD/SMI, DV.
• Experience facilitating group sessions or community events for women.
• Understanding barriers: safety, childcare, transportation, distrust.
• Strong organizational skills; manage caseloads and metrics.
