📋 Description

• Lead CCPM and TOC programs delivering longitudinal, patient-centered care.
• Oversee daily care management operations and proactive care strategies.
• Ensure adherence to clinical pathways and reduce complications.
• Drive KPIs and quality metrics; coach and develop the team.
• Build relationships with stakeholders for cross-program outcomes.
• Maintain clear communication with market leaders to improve outcomes.

🎯 Requirements

• Bachelor's degree in healthcare or nursing (BSN required)
• Active state nursing license in good standing
• 3+ years longitudinal clinical nursing care management experience
• 2+ years in a clinical nursing managerial or leadership role
• MS Office Suite (Outlook, Teams) proficiency
• Ability to work remotely with dedicated space and reliable internet
