Program Director

📋 Description

• Plan and oversee PACE functions: hire, train, supervise staff, and ensure workflows.
• Plan and facilitate IDT and other participant-care meetings; document follow-ups.
• Oversee staff to drive quality care and adherence to SOPs and regulatory requirements.
• Collaborate on participant services: home care, home health, transportation, call center.
• Oversee site operations, supplies, equipment, maintenance, and vendor oversight.

🎯 Requirements

• Bachelor’s in health admin/related field; 5+ years mgmt exp; master’s preferred.
• Experience in nursing, social work, psychology, or rehab with frail elderly at home.
• 7+ years exp; 3+ in supervision; managing a senior health care program.
• Multidisciplinary mgmt exp (nursing, social work, rehab).
• Value-based care exp preferred.
• CPR and First Aid cert within 180 days of hire; cert preferred at hire.
