- Perform in-person or telehealth psychiatric assessments with the IDT to develop care plans.

- Refer participants to a psychiatrist as indicated in consultation with physicians.
- Oversee nursing home participant care with regular visits per WelbeHealth standards.
- Serve as IDT consultant, communicating changes and coordinating 24-hour care delivery.
🎯 Requirements
- Master's in Nursing or Physician Assistant with PMH emphasis
- PMH NP or PA certification
- Active PMHNP/PA state license without restrictions
- CDS certification and DEA license
- NPI license
- Reliable transportation
- 2+ years experience with frail/elderly
- CPR and First Aid cert within 180 days (preferred at hire)
