Provide continuous medical coverage and in-person assessments (initial/semi-annual/unscheduled)
Collaborate with Physicians and IDT to develop care plans and manage discharge planning
Assist in maintaining participant care in skilled nursing facilities and residences, with regular visits
Review outcomes and utilization data to improve practices and policies

🎯 Requirements

Graduation from an accredited school of nursing
NP or PA certification with prescriptive authority
RN state license
2+ years of experience with frail or elderly populations
Knowledge of chronic care and geriatric issues
Reliable transportation
Willingness for rotating night/weekend call
