Collaborate with Physicians and IDT to develop care plans and coordinate care.
Assist with discharge planning from acute or long-term settings.
Support care in skilled nursing facilities and residences with regular visits.
Review outcomes and utilization data to improve practices.

🎯 Requirements

Graduation from an accredited school of nursing.
NP/PA certification with prescriptive authority.
RN state license.
Minimum two years' experience with frail or elderly populations.
Knowledge of chronic care, geriatrics, and best practices.
Reliable means of transportation.
Able and willing to rotate night and weekend call.
