- Provide continuous medical coverage and in-person assessments (initial/annual/unscheduled)

- Assist with discharge planning from acute or long-term settings
- Maintain participant care in skilled nursing facilities and residences with regular visits
- Participate in rotating night and weekend call
π― Requirements
- Graduation from an accredited school of nursing
- NP or PA with prescriptive authority
- RN state license
- Minimum of two (2) years of experience with frail or elderly populations
- Knowledge of chronic care, geriatric issues, and best practices
- Reliable transportation
- Able and willing to rotate night and weekend call
