Provide ongoing medical care and in-person assessments (initial, semi-annual, unscheduled)
Collaborate with Physicians and IDT to develop care plans and coordinate care
Assist in maintaining participant care in SNFs and residences with regular visits
Review outcomes and utilization data to improve practices and policies

🎯 Requirements

Graduation from accredited school of nursing
NP or PA with prescriptive authority
RN license
Minimum 2 years with frail or elderly populations
Knowledge of chronic care and geriatric issues
Reliable transportation
Able and willing to rotate night and weekend call
