Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Collaborate with IDT to complete assessments and plan primary care.
• Educate WelbeHealth participants and families.
• Provide continuous medical coverage and in-person assessments.
• Develop participant care plans and coordinate care across teams.
• Arrange discharge planning from acute or long-term settings.
• Visit participants in skilled nursing facilities and homes as needed.

🎯 Requirements

• Graduation from an accredited school of nursing.
• NP or PA with prescriptive authority.
• RN license in the state.
• Minimum 2 years with frail or elderly populations.
• Knowledge of chronic care and geriatrics.
• Willing to rotate night and weekend call.
