Provide continuous medical coverage and in-person assessments (initial, semi-annual).
Collaborate with physicians and the IDT to develop care plans and coordinate care.
Assist with care in skilled nursing facilities and at patient homes.
Review outcomes and utilization data to improve practices.

🎯 Requirements

Graduation from an accredited school of nursing
NP or PA with prescriptive authority
RN state license
Minimum of two years' experience with frail or elderly populations
Knowledge of chronic care and geriatric issues
Reliable transportation
