Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide life-changing, community-based primary care to vulnerable seniors.
• Build deep, long-term relationships with patients in a value-based model.
• Work within the PACE program to deliver holistic care.
• Schedule 8am-5pm, Mon-Fri; includes home visits.
• On-call rotation few nights or weekends per month (phone only).
• Salary: $145,399 - $174,478 USD plus quarterly bonus.

🎯 Requirements

• Licensed NP or PA with 2+ years in elderly care.
• Prescriptive authority and DEA registration; Medicare/Medi-Cal eligible.
• CPR and First Aid certification within 180 days; ongoing.
• Strong communication and teamwork; collaborate with interdisciplinary team.
• Willing to perform home visits and adapt to value-based care.
