Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide primary care to vulnerable seniors in a community-based PACE program.
• Build long-term relationships with a small patient panel within an interdisciplinary team.
• Deliver value-based, holistic care focused on patient outcomes.
• Regular hours 8am-5pm, Monday-Friday, including home visits.
• Rotating on-call schedule a few nights or weekends, by phone only.

🎯 Requirements

• Licensed Nurse Practitioner or Physician Assistant with 2+ years elderly care experience.
• Prescriptive authority and DEA registration; eligible for Medicare/Medi-Cal.
• CPR and First Aid certification within 180 days of hire (preferred).
• Home visits experience and ability to travel to patient homes.
• Strong patient-centered, team-based, value-based care approach.
• Current licensure and eligibility to work in the U.S.
