Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Deliver community-based, value-based care to seniors.
• Build long-term patient relationships with holistic care.
• Collaborate on an interdisciplinary primary care team.
• Regular hours 8am–5pm, Mon-Fri, with home visits.
• On-call rotation a few nights/weekends by phone.
• Focus on compassionate, high-quality senior care.

🎯 Requirements

• Licensed NP or PA with prescriptive authority
• 2+ years experience with elderly patients
• DEA registration and eligibility for Medicare/Medi-Cal
• CPR and First Aid certification within 180 days of hire
• CPR/First Aid certification at time of hire preferred
• Willing to maintain ongoing certification
