Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide community-based primary care to vulnerable seniors
• Build long-term relationships with a smaller patient panel
• Deliver holistic care through WelbeHealth's value-based PACE program
• Collaborate with an interdisciplinary team
• Rotating on-call schedule, few nights/weekends by phone

🎯 Requirements

• Licensed NP or PA with current credentials
• 2+ years of elderly care experience
• Prescriptive authority with DEA registration
• Eligible to serve Medicare/Medi-Cal
• CPR and First Aid certification within 180 days of hire
