Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Deliver primary care to elderly patients in a value-based model
• Build long-term patient relationships with a smaller panel
• Conduct home visits as part of care delivery
• 8am-5pm, Mon-Fri; rotating on-call by phone
• Collaborate with interdisciplinary team for holistic care
• Support mission with patient-centered, outcome-focused care

🎯 Requirements

• Licensed NP or PA with prescriptive authority
• 2+ years' experience with elderly patients
• DEA registration; Medicare/Medi-Cal eligible
• CPR and First Aid certification within 180 days
• Ongoing CPR/First Aid maintenance
