Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide community-based care to vulnerable seniors.
• Build long-term relationships with a smaller patient panel.
• Deliver holistic, value-based care through the PACE program.
• Collaborate with an interdisciplinary team.
• Regular hours 8am-5pm, Mon-Fri; includes home visits.
• Rotating on-call schedule, few nights or weekends, by phone.

🎯 Requirements

• Licensed NP or PA with at least 2 years of elderly care.
• Prescriptive authority and DEA registration.
• Eligible to serve Medicare/Medi-Cal participants.
• CPR and First Aid certification within 180 days of hire.
• Maintain CPR/First Aid certification thereafter.
