Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide life-changing, community-based care to seniors.
• Build long-term relationships with a smaller patient panel.
• Deliver holistic, value-based care through the PACE program.
• See patients during regular hours; home visits included.
• Coordinate care with an interdisciplinary team.
• Regular hours 8am-5pm, Mon-Fri.

🎯 Requirements

• Licensed NP or PA with 2+ yrs elderly care.
• Prescriptive authority and active DEA registration.
• Eligible to serve Medicare/Medi-Cal.
• CPR and First Aid within 180 days of hire; certification on hire preferred.
