Primary Care Provider, Nurse Practitioner / Physician Assistant

📋 Description

• Provide community-based care to vulnerable seniors.
• Work in an interdisciplinary team within the value-based PACE program.
• Build long-term relationships with a smaller patient panel.
• Deliver holistic, comprehensive care to patients.
• Rotating on-call schedule by phone (nights/weekends).
• Regular hours 8am-5pm, Mon-Fri; includes home visits.

🎯 Requirements

• Licensed NP or PA with 2+ years geriatric experience.
• Prescriptive authority with DEA registration; eligible for Medicare/Medi-Cal.
• CPR and First Aid certification within 180 days of hire and maintained.
