Primary Care Physician, Pod Leader

📋 Description

• Provide community-based care to vulnerable seniors with an IDT.
• Lead comprehensive in-person assessments to personalize care plans.
• Coordinate discharge for acute, short-term, and long-term placements.
• No hospital rounds; focus on ~8 patients per day.
• Regular hours 8am-5pm Mon-Fri with home visits.
• Rotating on-call schedule by phone (nights/weekends).

🎯 Requirements

• Bilingual in Spanish and English required.
• Residency completed (ACGME-accredited).
• Master's or Doctorate degree.
• Active state physician license without restrictions.
• CDS certification and DEA registration.
• CPR/First Aid within 180 days (on hire preferred).
