📋 Description

• Perform comprehensive in-person assessments with the IDT to tailor care plans.
• Coordinate discharge for acute, short-term, and long-term placements.
• No hospital rounds; see about 8 patients per day.

🎯 Requirements

• Residency program accredited by ACGME
• Master's or Doctorate degree
• Active unrestricted state license; CDS and DEA registration
• CPR/First Aid certification within 180 days
• Specialties: Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics
• Medicare/Medi-Cal enrollment eligible
