Primary Care Physician, Pod Leader

📋 Description

• Provide comprehensive, community-based primary care for vulnerable seniors.
• Build long-term relationships with a smaller patient panel.
• Deliver holistic, value-based care within the PACE program.
• Perform in-person assessments with the interdisciplinary team to plan care.
• Coordinate discharge and placement for acute and long-term needs.
• No hospital rounds; about 8 patients per day with home visits.

🎯 Requirements

• ACGME-accredited residency required.
• Master’s or Doctorate degree.
• Board Certification preferred.
• Active, unrestricted state medical license.
• CDS certification and DEA registration.
• CPR and First Aid certification within 180 days; hire-ready CPR/First Aid preferred.
