Primary Care Physician, Pod Leader

📋 Description

• Provide life-changing, community-based care to vulnerable seniors.
• Deliver holistic care within a value-based PACE program.
• Role includes Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics.
• Collaborate with the IDT to create personalized care plans.
• No hospital rounds; focus on comprehensive care, 8 patients per day.

🎯 Requirements

• Residency from an ACGME-accredited program.
• Master's or Doctorate degree.
• Board Certification preferred.
• Active state physician license without restrictions.
• CDS certification and DEA registration.
• CPR and First Aid certification within 180 days of hire.
