📋 Description

• Perform comprehensive in-person assessments with the IDT to develop a personalized care plan.
• Coordinate discharge for acute, short-term, and long-term placement.
• No hospital rounds; focus on comprehensive care; about 8 patients daily.

🎯 Requirements

• Residency program accredited by the ACGME.
• Master's or doctorate Degree.
• Board Certification preferred.
• Active state physician license without restrictions.
• State CDS certification and DEA registration.
