Practice Transformation Specialist

📋 Description

• Implement products and ACO initiatives with Market Leadership guidance.
• Ensure practices meet goals in Transformation Plans (wellness visits, transitions, active management).
• Train office staff and physicians on processes and technologies; assess training needs.
• Develop relationships with physicians, providers, and executives to improve financial and quality metrics.
• Collaborate with teams to address concerns and identify opportunities for improvement.

🎯 Requirements

• Must be resident of California.
• Graduate of an accredited college or university; 4–6 years in medical practice management may substitute.
• 3 years of experience with Electronic Health Records (EHR) in clinical/practice management.
• Excellent oral and written communication, organizational and project management, and team-building skills.
• Familiarity with process mapping and workflow analysis tools.
• Excellent computer skills and willingness to learn additional software.
