• Implement ACO initiatives per Market Leadership guidance.
• Support wellness visits and transitions of care.

🎯 Requirements

• Must be resident of Oregon.
• Bachelor's degree or equivalent; 4–6 years in medical practice management.
• 3+ years of Electronic Health Records (EHR) experience.
• Excellent communication, organizational, and project management skills.
• Familiarity with process mapping and workflow analysis tools.
• Strong computer skills; willingness to learn new software.
• Ability to travel to Oregon practices ~25% of the year.
