📋 Description

• Implement ACO initiatives with Market Leadership direction.
• Lead wellness visits, care transitions, and chronic care mgmt programs.
• Train staff and physicians on processes and the Aledade curriculum.
• Build relationships with physicians, providers, and executives to boost performance.
• Listen to practice concerns to identify opportunities for improvement.

🎯 Requirements

• Must be resident of Oregon.
• Graduate of an accredited college or university; 4-6 years in medical practice mgmt may be accepted.
• 3 years of EHR experience for clinical/practice mgmt.
• Excellent communication, organizational, project mgmt, and team-building.
• Familiarity with process mapping and workflow analysis tools.
• Sitting for prolonged periods; extensive computer use.
• Travel to practices within southern Oregon ~25% across year.
